Abstract

Objective : The present study is aimed to study the utilization of anti-diabetic agents in the Department of Nephrology among hospitalized diabetic patients with CKD.

Methodology:  The study was designed as a prospective observational in a multi-specialty tertiary care hospital in Bangalore, India. Detailed patient information was obtained from the patient’s case sheet, including patient demographics, diagnosis, and history of medical and medication details. The laboratory data details like blood pressure values, blood glucose values, etc. were noted down, documented in a suitably designed patient data collection form, and analyzed for utilization patterns concerning anti-diabetic drugs.

Result:  Out of 90 patients, (79%) were male and (21%) were female. Maximum patients belonged to the age group of (50-59) years. The most frequently used non-insulin medication classes included biguanides (metformin), sulfonylureas, thiazolidinediones, and DPP-4 inhibitors. Insulin is the oldest and highly used of the currently available medications and, therefore, the treatment with which we have the most clinical experience. Among the available oral antidiabetic drugs, sulfonylureas and glinides are associated with a risk of hypoglycemia.

Conclusion: The use of particular antidiabetic medications in patients with CKD changed noticeably over the years, most following guidelines and regulatory decisions. Gaps in the quality of care still exist, which warrants increasing awareness and implementing programs to mitigate inappropriate use.
